Kat is a Colorado native who has had close ties to Family Promise of Colorado Springs since 2013, when she and her children experienced homelessness. Six years later, she now serves as the executive director of the same affiliate that served her and her children, and she sits on the Guest Advisory Council with Family Promise National. Kat was empowered to overcome her homeless situation with the support and services of Family Promise and its amazing volunteers.

Job Title: Case Manager – Rapid Rehousing & Aftercare
Reports To: Executive Director
FLSA Status: Exempt

Summary: This position is responsible for providing client-centered case management services involving a great deal of direct service interaction, partnering with various stakeholders, and system collaboration for our Rapid Rehousing and Aftercare programs.
Essential Duties and Responsibilities:
1. Case Management
-Document work in case files (and other methodologies as appropriate, including but not limited to HMIS, spreadsheets, etc.) according to agency protocol, to meet or exceed requirements set by grantors and professional ethics guidelines.
-Document all client contact, including case management meetings, emails, and phone conversations.
-Create mutually agreed upon goals and track/document via goal sheets helping clients accomplish and work towards self-sufficiency.
-Complete monthly budgets with clients and work with client to adhere to the determined budget.
b. Rapid Rehousing
-Complete required NC DHHS Emergency Solutions Grant (ESG) timesheet/billing forms.
-Manage caseload of maximum 15 families.
-Landlord engagement: (1) Recruiting, building rapport, and managing retention of landlords/property managers, (2) Quarterly check in with established landlords/property managers.
-Housing Search & Placement: (1) Meet with clients minimum 2 scheduled times per month, (2) Work with client through consumer choice to determine best options for permanent housing.
-Housing Stabilization: (1) Meet with clients minimum 1 scheduled time per month, (2) Monthly check in to assure rent/utilities are paid.
-Submit financial assistance payment requests to Executive Director in timely manner.
-Oversight and coordination of aftercare program, including linking families to community resources while in program and after transitioning to permanent housing. Includes, but not limited to, maintaining Facebook page and facilitating special events.
-Manage caseload of former clients that agree to engage in aftercare services, including at least one face to face scheduled meeting per month.
-Complete monthly check-ins with all clients that have exited during previous year.
-Coordinate and facilitate two events per month; one life skill class and one peer support group.
2. All Direct Care Staff Responsibilities
-Participate in mandatory on call rotation for support to congregations, Day Center volunteers, and residents.
-Participate in rotation of Monday night shifts to provide Day Center coverage and assist volunteers.
-Communicate with host congregations through van sheets, host info, family updates, etc.
-Act as task supervisor to minimum one intern.
-Develop and maintain assigned strategic external partnerships and represent agency at relevant community meetings and events.
-Verify proper and complete data collection on program outputs and outcomes; maintain expertise in NC HMIS use and reporting.
-Attend staff, Board committee, and community meetings as directed. Participate in staff presentations and/or represent agency in community when needed. Cooperate with volunteer involvement, work teams, and donation drives.
*Other duties as assigned.
Qualifications: A bachelor's degree in a human service field or comparable work/life experience required. One year of experience working with populations experiencing homelessness required. Successful experience in human services program delivery and management, especially with families, is primary qualification. Strong assessment skills, case management experience, and the ability to function as part of a team preferred. Proficiency with MS Office tools (Word, Excel) and Google Suite (Gmail, Calendar, Drive) is essential. Ease in data entry in required internet based database required. Fluency or facility in Spanish is a plus.
Working Hours and Conditions:
-Full-time: 10 days on (Monday thru following Wednesday), 4 days off rotation (Thursday through Sunday).
-Required participation in the 24 hour on-call rotation including nights, weekends, and holidays.
-Must be willing to work a flexible schedule, including holidays.
-Position requires a combination of office-based and community-based duties for meeting/events and service delivery.
-Must conduct client home visits.
-Valid North Carolina Driver’s License and satisfactory driving record
-Current valid proof of car insurance
-Acceptable background report
Compensation: Competitive salary with generous benefits, 403(b) plan available.
Here at Family Promise of Wake County, we have so much for which we are thankful! Whether its a volunteer at one of our churches comforting a newly homeless child or a caseworker counseling a father as he prepares to move his daughters out of transitional housing and into their own apartment, everything we do for our families starts with our supporters.

September! Remember when that meant a fresh, new year at school? Maybe you remember shopping for a new book satchel, trapper keeper or backpack. How about sharpened pencils, packs of paper or notebooks and, just maybe, new school shoes or a new outfit? Of course there was always anxiety and uncertainty mixed with the excitement. After all, there was the unknown of a new teacher and a new mix of classmates.
Twenty two years ago I visited San Francisco and saw homelessness like I had never seen before. It was then I made a promise to God that I would make helping those experiencing homelessness my commitment to Him.
This week’s report from the National Low Income Housing Coalition shows what anyone who works with those experiencing homelessness already knows – the lack of affordable housing in this country is staggering. The coalition found there is no place in the United States where a worker earning minimum wage could afford a modest two-bedroom rental. In North Carolina, it would take 90 hours of minimum-wage work each week – and in Wake County 109 hours – to cover the cost.
Whether you're trying to come up with yet another way to use leftover turkey or wrestling decorations out of the attic, the transition from fall to winter is a time of anticipation and celebration for many families. Not everyone in Wake County, however, has a warm kitchen in which to prepare food or a home in which to store their treasured holiday memories. Today, on Giving Tuesday, we ask you to take a moment during your preparations to remember those who need your help this holiday season.
Thanks to your immense generosity, we did it! We raised enough money in eight days to cover all of the damages and necessary repairs associated with our flooded well house and the fallen tree that damaged two of our families' cars. (If you missed the story of our very bad week, you can read about it on our blog.)
While we’re usually celebrating positive milestones in the lives of our Family Promise guests, right now, we urgently need your help after two significant crises in less than a week.
